KGP Group Inc., doing business as SpeQtrum Prepress Production Services, is facing 14 serious and one other-than-serious safety violation at its facility on Stanley Drive in Euless, TX, Occupational Safety and Health Administration officials said.

OSHA’s Fort Worth Area Office initiated an inspection Dec. 2 under the agency’s Site-Specific Targeting Program, which focuses enforcement efforts on industries with high injury and illness rates. Proposed penalties total $44,800.

The serious violations involve failing to keep exit areas unobstructed, properly storing compressed gas cylinders, providing proper machine guarding, ensuring forklifts undergo proper servicing and maintenance, providing forklift operator training, addressing electrical wiring deficiencies and properly mounting fire extinguishers. A serious violation occurs when there is substantial probability that death or serious physical harm could result from a hazard about which the employer knew or should have known.

“This company exposed its workers to ‘struck-by’ hazards associated with forklift operations, possible amputations and other injuries from the lack of machine guarding, and shocks from poor electrical wiring,” said Jack Rector, OSHA’s area director in Fort Worth.

The other-than-serious violation is failing to complete the OSHA 300 log of injuries and illnesses in detail as required. An other-than-serious violation is one that has a direct relationship to job safety and health, but probably would not cause death or serious physical harm.

SpeQtrum Prepress Production Services manufactures printing plates and cutting dies, and employs 35 workers at the Euless location.
