The U.S. Chemical Safety Board (CSB) released findings of the interim investigation into the leak of a toxic chemical at the DuPont facility in La Porte, TX. Four employees died and three others suffered injuries in the Nov. 15, 2014 incident.

A video from the CSB showed how a series of errors that began five days earlier led to the release of nearly 24,000 pounds of methyl mercaptan, a toxic chemical that killed the four workers at the DuPont manufacturing facility.
