Metalsa Structural Products Inc. in Hopkinsville, KY, is facing a repeat citation for exposing workers to safety hazards, said officials at the Kentucky Occupational Safety and Health (OSH).

Kentucky OSH inspectors found the auto metal stamping plant failed to ensure proper lockout/tagout procedures were followed while workers performed maintenance on a robotic machine, exposing them to amputation hazards.

Inspectors also found the company failed to conduct inspections on the control of hazardous energy and attach lockout/tagout devices on machinery.

The company is facing $35,000 in fines.

The company was previously cited for violations of these standards in 2013.
